get a big score. Dizzy the boss to raise your multiplier and don't get dizzy yourself
What screen recording software do you use? All of the things that I've tried add large amounts of lag to the game, thus meaning that my result is choppy and hard to follow.

On Barbarian Bob you can just jump over enemies, run and jump right before their shield.
When you do red moon medal orb hunter, if you jump kill yourself to get some red orbs, are they counting or do you have to get them without dying to complete the medal?
What I did was find a level with 2 or 3 orbs close by, collected them and restarted the level over and over till I got the medal

there's nothing to "make" for it. All you do is shoot for hours. You're probably not looking at the in game achievements and thinking the ng medals are what you need to unlock to get artifact 19.
For future reference I think a good way of doing the ground spin is getting him dizzy by reflecting the single cannonballs, then once that occurs just mash the hell out of the a button.

jump off the ledge on the level with the big column after you killed all the guys in that level. Then keep doing it till you get the medal

To answer Richler's question, after receiving the vial shoot Milos, then when it is converted into a bomb, give it to the worm before he can give you the key. Walk into the terminal waiting room, and you will be shot by a robot, and the medal will be yours.

What's a good arrow combination for Quickdraw? Lightning-Explosion worked for me, but my aim is so bad I've found Explosion-Homing better.
